The Bulgarian Red Cross Youth have its own structure, which is organized in 270 clubs, distributed in 28 regional organizations in the country, coordinated by the national leadership of the Bulgarian Red Cross Youth. Thanks to the well-established three-tier structure of the organization in the country, the Bulgarian Red Cross Youth can successfully implement its activities and reach as many people as possible. Anyone between the ages of 15 and 29 can be a member of the organization and participate in BRCY clubs, which are also the foundation of the BRCY structure. Clubs can be organized in schools, depending on the location or interests. Each club is managed by a club coordinator and deputy-club coordinator, who are elected during the club meeting from among all members. In addition to the mentioned clubs, there are also the so-called "Friends of the Bulgarian Red Cross Youth", in which young people up to 14 years of age, can be volunteers, and after reaching the required years to become members of the organization and be elected to leadership positions. There are BRCY clubs in all 28 districts in the country. Each district organization is led by a district coordinator and deputy district coordinators, elected during the General meeting of the clubs, which is attended by club coordinators and elected representatives from all clubs in the district.

The Supreme Forum of the Bulgarian Red Cross Youth is the General assembly of the organization. It is attended by representatives of all 28 regional organizations in the country, who elect a President of the BRCY, members of the Executive board of the BRCY and a Compliance Advisor. 

The President of the Bulgarian Red Cross Youth represents the organization and is a member of the General Assembly of the Bulgarian Red Cross. The Executive board of the BRCY is the main executive and administrative body of the organization. The Executive board of the Bulgarian Red Cross Youth run the organization between the General assembly. The Compliance Advisor controls the organization and monitors whether the decisions of the Executive Board comply with the current organizational documents.

Each of the leading positions at club, regional and national level has a two-year term.
